Fill out our secure intake form to get started. Once submitted, you’ll receive an email invitation to set up your CharmHealth Portal. In the portal, you’ll complete your profile, add payment details, and upload any recent labs (CBC, CMP, HA1C encouraged) for the provider.

After registering, you’ll schedule a virtual assessment with one of our RNs or licensed providers. Once your visit is booked, your required consent forms will be sent to your portal. Complete these forms before your appointment to ensure your provider has everything they need.

After your telehealth visit, one of our providers will review your intake, medical history, and lab results. If appropriate, a prescription will be issued through our partner pharmacy. Your medications will then be shipped directly to your door with continued support from our team.
