Geriatrics care

BYCARE is available to visit residents in their homes, assisted living facilities, and skilled nursing facilities. We understand the importance of delivering patient care in a familiar environment for various reasons. 

Below are the key responsibilities of our providers:  

• Work proactively to prevent patient hospitalization by assessing and treating changes in condition. 
• Collaborate closely with the attending physician to monitor medication use.
• Partner with facility staff to manage patients experiencing weight loss, pain, wound care, and the use of antibiotics and IVs. 
• Conduct comprehensive assessments to support high-quality patient care.
•  Engage with the patient’s family during the initial visit to discuss the care approach.
• Make monthly calls to family members to provide updates on the resident’s status as needed for each individual. 
• Address advanced care directives when appropriate.
