Geriatrics care

BYCARE is available to visit residents in their homes, assisted living facilities, and skilled nursing facilities. We understand the importance of delivering patient care in a familiar environment for various reasons. 

Below are the key responsibilities of our providers:  

1. Work proactively to prevent patient hospitalization by assessing and treating changes in condition. 
2. Collaborate closely with the attending physician to monitor medication use.
3. Partner with facility staff to manage patients experiencing weight loss, pain, wound care, and the use of antibiotics and IVs. 
4. Conduct comprehensive assessments to support high-quality patient care.
5.  Engage with the patient’s family during the initial visit to discuss the care approach.
6. Make monthly calls to family members to provide updates on the resident’s status as needed for each individual. 
7. Address advanced care directives when appropriate.
